The requirements of a Class I certified contractors include, but are not limited to, having a licensed professional engineer and/or professional geologist by the State of South Carolina on staff, plus the following insurance requirements:
The information on the application is used to determine the qualifications of the application to perform remediation activities for the Division as either a Class I contractor or a Class II contractor.
The W-9 form provides the applicable identification information that is used to process payments for performance for the Division that includes the mailing address and taxpayer identification number.
Costs that have been denied based on incomplete work, missing documentation, work not approved, etc. are subject to review. The denied cost will be included in the payment notification letter distributed by the Finance Section of the Division with a time frame of thirty days to submit a request for review (RFR) (pdf) .
