For Septic Installers

• Final Inspection Form
  • Licensed Septic System Installers who perform a self-inspection must submit the Final Inspection Form (DES 4432) to the Department within two (2) business days of completing the system installation. The forms should be submitted to BRLS-OSWW-Field-Admin@des.sc.gov. Please note the county name in the email subject line. Site drawings included on the form should reflect the actual installation. Do not use the drawing included on the Permit to Construct. Make sure that the applicable installation specifications are completed and identified measurements are documented on the site drawing. Incomplete self-inspection forms will delay the issuance of an Approval to Operate.
