SCDES is now offering an on-line platform for submitting approval to operate (ATO) requests for onsite wastewater systems. You will be able to use the electronic ATO form to schedule your final inspections. 

Once the form is submitted, you will receive a confirmation email notifying you that the request has been received. If the requested time is not available, SCDES staff will contact you. We hope this will make the scheduling process easier and afford you the ability to schedule a final inspection at your convenience. As a reminder, final inspection requests must be submitted prior to 10 AM the business day before the desired date of inspection. All self-inspection forms must be submitted within 48 business hours after the scheduled date of inspection.

Final Inspection Form

Licensed Septic System Installers who perform a self-inspection must submit the Final Inspection Form (DES 4432) to the SCDES within two (2) business days of completing the system installation. 

  • The forms should be submitted to BRLS-OSWW-Field-Admin@des.sc.gov.
  • Please note the county name in the email subject line.
  • Site drawings included on the form should reflect the actual installation. Do not use the drawing included on the Permit to Construct.
  • Make sure that the applicable installation specifications are completed and identified measurements are documented on the site drawing. 

Incomplete self-inspection forms will delay the issuance of an Approval to Operate.
