The information on the application is used to determine the qualifications of the application to perform remediation activities for the Division as either a Class I contractor or a Class II contractor.
The W-9 form provides the applicable identification information that is used to process payments for performance for the Division that includes the mailing address and taxpayer identification number.
A site check involves sampling soil, groundwater, or a combination of both, from an area where a release may have occurred. When the Division suspects that a release may have occurred from any portion of a UST system, a site check will be required. Releases most often occur from dispensers or spill buckets.

Costs that have been denied based on incomplete work, missing documentation, work not approved, etc. are subject to review. The denied cost will be included in the payment notification letter distributed by the Finance Section of the Division with a time frame of thirty days to submit a request for review (RFR) (pdf) .

A marina is defined as a land-based fueling facility that dispenses fuel over or in close proximity to a waterway. South Carolina currently has 71 marinas that are regulated by the Underground Storage Tank (UST) Management Division (44 saltwater and 27 freshwater).
Because marinas are special facilities that have concerns that differ from "normal" gas stations, there are certain points that should be considered any time a marina is installed or updated.

On July 1, 2011, the Underground Storage Tank (UST) Management Division implemented a Quality Assurance Program Plan (QAPP). The goal of the QAPP is to formalize procedures that will provide accountability and quality control for the data collected at UST sites.
Contractors must complete and submit Appendix B of the QAPP (Contractor Addendum) when requested for site-specific scopes of work (e.g. groundwater sampling and monitoring well installation). The UST Management Division will request the QAPP Addendum when an appropriate scope of work is determined to be necessary.
